内容提要:
Background: Platform screen doors effectively prevent railway falls and suicides, but high costs impede widespread implementation. This study evaluates the cost-effectiveness of installing platform screen doors along Hong Kong’s East Rail Line and explores affordable partial installation alternatives based on incident characteristics.
Methods: Data on platform-fall incidents from 2014 to 2022 (n = 96) were obtained from the MTR Corporation. Spatial and demographic patterns were examined using Poisson and binomial tests. We evaluated the cost-effectiveness of the recent installation project using the incremental cost-effectiveness ratio, defined as the cost per disability-adjusted life year averted over a 25-year time horizon.
Results: The installation project along the East Rail Line is estimated to avert 130.06 disability-adjusted life years annually. The incremental cost-effectiveness ratio is 28,689 US dollars per disability-adjusted life year, meeting the World Health Organization cost-effectiveness criterion by the eleventh year. Spatial analysis revealed that severe injuries were significantly concentrated at the end of the platform where trains enter stations. Based on this finding, we explored an alternative installation strategy that covers only the first one-third of each platform. Compared with a full installation, this partial strategy could still prevent approximately 78% of disability-adjusted life year losses while improving cost-effectiveness by about 15 %, at an estimated 24,530 US dollars per disability-adjusted life year.
Discussion: Platform screen door installation is highly cost-effective in Hong Kong's dense urban rail network. Where financial resources are limited, targeted installations in high-risk platform zones offer an economically efficient, viable alternative.
